Registered Nurse Case Managers

Our highly skilled registered nurses have an abundance of experience in the home care setting. They are able to fully assess all of the needs of the patient's and effectively coordinate their care.

The case manager serves as an important link between the physician and the patient. Together, the physician and case manager are able to ensure that the patient maintains independence while recovering at home.

Physical, Occupational & Speech Therapy

Our therapy department is comprised of a large full time staff of highly qualified therapists who possess exceptional skills for treating the needs of home bound patients. The goals that our therapists strive to achieve maximize the independence necessary for functional mobility and activities of daily living.

Medical Social Workers

Our compassionate and resourceful staff of medical social workers assist patients and their families/caregivers with emotional, social and financial issues. They will develop a personalized treatment plan to meet the patient's individual short and long term goals. Some of the information and support they provide is on the following:

  • Long range planning
  • Disease process
  • Stress management/coping skills training
  • Individual counseling
  • Altered body image
  • Support Groups

Certified Home Health Aides

All of our home health aides are state certified through the Hospice and Homecare Foundation. They follow the plan of care developed by the case manager. Some of the functions they perform are:

  • Monitor vital signs and report changes
  • Assist with simple wound care
  • Follow range of motion exercises as developed by the case manager
  • Assist with personal care: bath, shower, shampoo, skin care
  • Prepare meals as necessary
  • Light housekeeping/laundry tasks
